The Home Care Packages program provides government-funded home care across four levels of support to help you keep your independence and lifestyle and live in your own home for longer.  Depending on your situation, you may be eligible for a Home Care package ranging from levels 1-4.  The difference between the levels is the amount of support you require to meet your needs, and the amount of funding you require to deliver the services to you.  When you apply for a Home Care Package, an assessment of your specific needs will determine the level of support you need and the amount of funding you receive.

Our dedicated team will help you apply for and receive a Home Care Package, understand your budget and know what you can spend this on.  Our friendly team can also help you with other support while you wait for your Home Care Package to be approved. This could be government or privately funded.

What is the Process?

At Heather Hill, we aim to make commencing Home Care Support as simple as possible.  From your first enquiry through to the start of your Care Solutions, your personal Care Coordinator will be with you each step of the way.

I already have a Home Care Package

Step 1 - Enquire

Contact Us online or call our Care Support Team on 07 3720 9122 to organise your complimentary Health & Wellbeing Consultation.

Step 2 - Health & Wellbeing Consultation

Either in the comfort of your own home, or on the phone, meet with your personal Registered Nurse Care Coordinator who will assist and advise you on accessing government funding.  After your consultation your Care Coordinator will provide a comprehensive Health & Wellbeing Summary outlining your funding options, and the steps necessary to obtain home care package funding.

Step 3 - Design Your Care Plan & Care Team

Your personal Care Coordinator will work with you to design a Care Plan with the best Solutions to enable you to remain independent whilst achieving a high level of wellbeing, as well as making the most of your allocated package.  Your experienced and qualified Care Team will be carefully selected to provide you certainty and consistency.

Step 4 - Home Care Supports Commence

Step 1 - Enquire

Step 2 - Health & Wellbeing Consultation

Either in the comfort of your own home, or on the phone, meet with your personal Registered Nurse Care Coordinator who will assist and advise you on accessing government funding.  After your consultation your Care Coordinator will provide a comprehensive Health & Wellbeing Summary outlining your funding options, and the steps necessary to obtain home care package funding.

Step 3 - Application Support & Assessment

Your Care Coordinator will support your applications with the Australian Government My Aged Care for a Home Care Package or other funding, making it as stress-free as possible to obtain home care assistance.

An Assessment will be organised through the government’s My Aged Care to determine which Home Care Package you are eligible for.  If you are eligible you will receive a confirmation letter confirming which Home Care Package you are eligible for, and your priority package level.

Step 4 - Interim Support & Package Allocation

Despite being approved for a Home Care Package, there can be considerable wait times (12 months+) for a Home Care Package to be assigned to you.  Your personal Care Coordinator will work with you to access other government funding or to arrange Private (Self-Funded) Care in the meantime.

Around three months before your package is allocated you will receive a letter from the Australian Government.  When your Package is allocated you will receive another confirmation letter.  Contact your Heather Hill Care Coordinator as soon as possible after receiving these letters, so your Care Plan can be designed.

Step 5 - Design Your Care Plan & Care Team

Your personal Care Coordinator will work with you to design a Care Plan with the best Solutions to enable you to remain independent whilst achieving a high level of wellbeing, as well as making the most of your allocated package.  Your experienced and qualified Care Team will be carefully selected to provide you certainty and consistency.

Step 6 - Home Care Supports Commence
