Heather Hill Group is a registered NDIS provider for participants across SE Queensland. Our team provides Community Access, Assistance with Self-Care Activities and Supporting Independent Living Solutions with participants. Choice and control are essential to those we work with to ensure you can live life the way you would like. Our support workers are skilled in ensuring that long friendships form part of daily tasks.
What is the NDIS?
The National Disability Insurance Scheme (NDIS) is a framework allowing people living with disability to access support across Australia. The NDIS focuses on providing choice and control to those it services to help improve independence, assisting daily living and improving skills.

NDIS Services provided by Heather Hill Group
Supported Independent Living (SILs)
Depending on your situation we can assist you with your existing household provider or support you at home.
Our support workers can be either 24/7 or provide daily program assistance to allow you to feel safe and comfortable.
Assistance with Social & Community Participation
Heather Hill Group’s Support Workers can help you with a range of supports to help you with your daily living at home or in the community. We are always happy to support you to try new social outings or go and catch up with family and friends.
Tasks we can assist with may include:
- Going on a day trip;
- Going with you to exercise or group activities;
- Going to buy groceries or shopping;
- Assisting chores around the house; and
- Companionship and social inclusion at home or out and about.
Assistance with Self-Care Activities
- Assistance with personal hygiene and care
- Help with bathing or showering
- Help dressing and undressing, including selecting clothes
- Help with dental hygiene

Our team will work with your coordinator and you to ensure we get the best fit for your plan. This means ensuring that your support worker is experienced to help you achieve your goals and has your interests and needs at the centre of everything.

At Heather Hill, we aim to make commencing support as simple as possible.
From your first enquiry through to the start of your Support Solutions, your personal Care Coordinator will be with you each step of the way.
Step 1 - Enquire
Contact Us online or call our Care Support Team on 07 3720 9122 to organise your complimentary Health & Wellbeing Consultation.
Step 2 - Health & Wellbeing Consultation
We will work closely with your NDIS Coordinator to provide the best support either in the comfort of your own home, or on the phone. Our friendly Community Coordinator will meet with you discuss your support plan. After the consultation our Community Coordinator will work with you or your NDIS Coordinator to best support you.
Step 3 - NDIS Supports Commence
Welcome to the Heather Hill family. Your supports will commence with the best possible matched support worker to begin with your journey.
